EAP Clinical / Case Manager

Location: Remote

Job Description:

• Provide telephonic assessment, consultation and referrals to program participants
• Offer in-the-moment guidance, advocacy and support
• Complete a Comprehensive Clinical Assessment, including identification of presenting issues(s) and screening for risk concern(s)
• Develop an Individualized Clinical Care Plan for program participants to address presenting issue(s) or work-life concern(s)
• Assist participants in locating and securing appointments for counseling
• Act as an advocate for the customer’s EAP services, knowing what is part of the customer’s contract
• When appropriate, coordinate with the most appropriate level of care including instructing the member on how to access care under the benefit plan
• Perform timely follow-up with participants and providers
• Measure the impact of EAP services on participant health, well-being and productivity
• Maintain accurate and complete records in compliance with HIPAA and ethical guidelines
• Assist with connecting members to legal and/or financial vender resources
• HR consultations
• Mandatory/Management referral coordination
• Perform other duties as assigned

Job Requirements:

• Master’s Degree in a mental health discipline (i.e. Psychology, Social Work, Counseling, Marriage and Family Therapy)
• Minimum 2 year's Employee Assistance Program (EAP) experience
• Fully Licensed (LCPC, LCSW, LMFT)
• Intermediate experience with video and phone counseling
• Intermediate experience working with adults, adolescents, and couples on issues including depression, anxiety, relationship concerns, job stress, grief, trauma, and alcohol/substance use
• Willing to obtain board certified-Telemental Health (BC-TMH) credential
• Willing to provide counseling services through phone, video, chat, and asynchronous texting
• Fundamental understanding of MS Office
• Strong reasoning ability – capacity to collect data, establish facts, define problems, and draw valid conclusions
• Familiarity with the most current edition of the Diagnostic and Statistical Manual of Mental Disorders
